CREDIT AND BACKGROUND CHECK AUTHORIZATION FORM

 

NewtekOne, Inc. (“Newtek”) offers a wide range of products and services (collectively, the “Newtek Services”) through its affiliates (collectively, including Newtek, the “Newtek Entities”), including without limitation, Newtek Bank, National Association (“Newtek Bank, N.A.”) and its wholly-owned subsidiary Newtek Business Lending, LLC, Newtek Business Services Holdco 6, Inc., Newtek Small Business Finance, LLC, Newtek Merchant Solutions, LLC, PMTWorks Payroll, LLC and Newtek Insurance Agency, LLC. 

By signing this Credit and Background Check Authorization Form (this “Authorization Form”) below, you hereby authorize the following (collectively, the “Authorizations”):

  • each of the Newtek Entities to perform credit and background checks from time to time on you and each of the companies in connection with which you are submitting this Authorization Form (collectively, the “Companies”), including without limitation obtaining consumer reports and credit reports and contacting credit reporting agencies and/or any other credit references for the purpose of verifying your and/or any Company’s personal or business credit history information, as applicable, in connection with your or any Company’s application for Newtek Services or the delivery of any Newtek Services to you or any of the Companies; 
  • any individual or entity, credit reporting agency or public agency (including the Social Security Administration and law enforcement agencies) to divulge any and all information, verbal or written, pertaining to you or any Company to any of the Newtek Entities and/or their agents, including any consumer reporting agency; and
  • the complete release of any records or data pertaining to you or any Company, which the individual, entity, credit reporting agency or public agency may have, to include information or data received from other sources.

By signing below, you also (1) acknowledge that you have read this entire Authorization Form and understand the same; (2) personally represent that you are authorized to provide all of the foregoing Authorizations with respect to each of the Companies; and (3) hereby, on behalf of yourself and each of the Companies, release each of the Newtek Entities from any and all liability relating to or arising from any of the Authorizations.
